As complicações relacionadas às feridas cirúrgicas, como infecção do sítio cirúrgico, deiscência, seroma e hematoma, permanecem entre as principais causas de morbidade após procedimentos abdominais, impactando negativamente a recuperação dos pacientes e elevando os custos assistenciais. Nesse contexto, a Terapia por Pressão Negativa (TPN) consolidou-se como uma importante estratégia terapêutica e profilática para o manejo de feridas cirúrgicas complexas e prevenção de complicações da parede abdominal. Este capítulo apresenta uma revisão narrativa estruturada sobre os fundamentos fisiológicos da TPN, seus mecanismos de ação, principais indicações clínicas e as evidências científicas relacionadas à sua utilização em cirurgia abdominal. São discutidas as aplicações da terapia em feridas abertas, abdome aberto, reconstruções da parede abdominal e incisões cirúrgicas fechadas de alto risco, destacando os benefícios observados na redução da infecção do sítio cirúrgico, deiscências, seromas, hematomas e necessidade de reintervenções. Também são abordadas as limitações, contraindicações e perspectivas futuras da técnica, considerando os avanços tecnológicos e a crescente incorporação da terapia aos protocolos cirúrgicos modernos. As evidências disponíveis demonstram que a Terapia por Pressão Negativa constitui uma ferramenta eficaz e segura para otimizar a cicatrização, reduzir complicações pós-operatórias e melhorar os desfechos clínicos em pacientes submetidos à cirurgia abdominal, especialmente naqueles com elevado risco para alterações da parede abdominal.
